Created in March 2004, this is the online repository for Reportable Accidents and, starting in May of 2006, for Minor Accident Narratives. Reportable accidents are those which involve either a personal injury or over $1000 worth of damage to one or more vehicles. Minor accident narratives are those without injuries and damage of less than $1000 to each vehicle.

Accident Reports will generally not be available here, by phone, or in person until 10-14 days after the accident occurred. Thank you for your patience. You will need your exact case number in order to download your report. Case numbers are in the following format 06-100123. You will need to type the number as issued and include the dash.

Accident reports are maintained in a PDF (adobe acrobat) format. You will need to have the adobe acrobat reader installed to view the report. It is available as a free download from www.adobe.com.


All Accidents must be reported if there is an injury or death, or property damage in excess of $1000 to anyone's property, or in excess of $200 to government property.  To Report An Accident, call the police.  DO NOT COMPLETE this Driver Report of Accident if a law enforcement officer completed a Wisconsin Motor Vehicle Accident Report.

COMPLETE this Wisconsin Driver Report of Accident if:
• There was $1000 or more damage to any one person's property
- OR -
• Anyone was injured
- OR -
• There was $200 or more damage to government property, other than vehicles.

If you need to complete a Driver Report of Accident Form, that form is available from the State Department of Motor Vehicles here

Reports from before March of 2004 need to be requested from the Records Division.
