The operator license permits an individual to serve alcohol on the premises of a licensed Class A or B establishment. At least one person must be licensed on the premises at all times. View the Section 4-10.

Please include a copy of proof of completion of the responsible beverage server course, or proof that the applicant has been a licensed operator in the State of Wisconsin within the past two years. A legible copy of a government-issued picture ID is also required. The cost for this license is $50.
