One Application For Both Services

In an effort to simplify the process, we have partnered with Curative Connections Transportation Service. You are able to complete one application to apply for both services.

Recertification

Recertification for approved applicants is required every three years.

How to Apply

To apply for our ADA Paratransit service, please follow these steps:

Step 1: Print one of the applications provided on this page or use the online application. If you're completing the online application please be sure to print the Medical Information Release Authorization (PDF) and the Request for Professional Verification (PDF).

Step 2: Complete your ADA Paratransit Application. You then can either.

Mail your completed application to:

Fax your completed application to 920-448-3461.

By completing the online application once you click submit your application is emailed to the Paratransit Division.

Step 3: Have your medical physician complete the Request for Professional Verification (PDF).
